How MassHealth pays for behavioral health
Which payer owns a MassHealth member's behavioral health depends on the member's plan. Members in an Accountable Care Partnership Plan or a managed care organization get behavioral health through that plan or its vendor. Members in the Primary Care Clinician (PCC) Plan or a Primary Care ACO get behavioral health through the Massachusetts Behavioral Health Partnership (MBHP).
Under the Behavioral Health Roadmap, MassHealth designated Community Behavioral Health Centers that offer same-day urgent care, ongoing outpatient treatment and 24/7 mobile and community crisis services. CBHCs bill under their own MassHealth model, which is different from a standard outpatient clinic.
- Check the member's plan type at every visit; members can switch between ACOs and MCOs.
- MBHP, ACO and MCO authorizations and billing rules are separate.
- Provider enrollment and updates run through POSC.
- CBHC encounters follow MassHealth's CBHC billing rules, not standard clinic rules.
Medicare rules for Massachusetts behavioral health providers
Medicare rules are federal, so they are the same in Massachusetts as everywhere else. What changes is the contractor: Massachusetts clinicians enroll and bill Part B through National Government Services (Jurisdiction K).
- Counselors and MFTs can bill Medicare. Since January 1, 2024, marriage and family therapists and mental health counselors (including LPCs) can enroll in Medicare and bill for their services, paid at 75% of the physician fee schedule amount.
- Home telehealth is permanent for mental health. Patients can receive mental and behavioral health telehealth at home, anywhere in the country, and audio-only is allowed when the patient cannot or does not want to use video.
- The in-person visit rule is delayed. The requirement for an in-person visit within six months before starting tele-mental health, and every 12 months after, is waived until January 1, 2028 under the Consolidated Appropriations Act of 2026. Broader Medicare telehealth flexibilities run through December 31, 2027.
- Incident-to under general supervision. Behavioral health services by auxiliary personnel can be billed incident-to a supervising practitioner under general supervision.
- Tele-prescribing of controlled substances. DEA and HHS extended telemedicine prescribing flexibilities through December 31, 2026.
- Medicare Advantage is different. MA plans set their own networks and prior authorization rules, so we verify each MA plan before the first visit.
Massachusetts commercial plan notes
Massachusetts has strong parity and access laws for state-regulated plans. The 2022 Mental Health ACCESS Act added required coverage, including an annual mental health wellness exam. Self-funded employer plans follow federal rules instead, so we confirm plan type before applying state protections.
Common behavioral health denials in Massachusetts
These are the patterns we see most often with Massachusetts behavioral health claims, and each one is preventable with front-end checks:
- Claims sent to the ACO or MCO for MBHP members, or the reverse.
- Authorization obtained from the wrong behavioral health payer.
- CBHC services billed like standard outpatient clinic services.
- POSC enrollment or service location data not matching the claim.
- Medicare crossover claims denied because the clinician is not enrolled in Medicare.
Core behavioral health codes we bill in Massachusetts
Psychotherapy codes are time based, so documented start and stop times or total minutes must support the code billed. State Medicaid programs and plans may also require HCPCS codes, modifiers or rate codes on top of these.
