Behavioral Health Billing Services in Arizona
Last reviewed October 6, 2026 by the Healthcare Logic revenue cycle team.
Arizona tightened behavioral health billing sharply after large-scale Medicaid fraud was uncovered in 2023. AHCCCS paused new enrollments for several behavioral health provider types, raised others to high-risk status and ended most retroactive enrollment. The pause is over, but the stricter enrollment and documentation rules remain.
Healthcare Logic supports Arizona therapists, group practices, outpatient clinics, integrated clinics and FQHCs with AHCCCS enrollment, authorizations, coding, claims, denials and credentialing.

How AHCCCS pays for behavioral health
Most AHCCCS members are enrolled in an AHCCCS Complete Care (ACC) plan, which covers physical and behavioral health together. Members determined to have a serious mental illness are served by ACC-RBHA plans. American Indian members can choose the American Indian Health Program (AIHP) or tribal programs, which are billed fee-for-service to AHCCCS.
From May 2023 AHCCCS placed a moratorium on new enrollment of behavioral health outpatient clinics, integrated clinics, behavioral health residential facilities and community service agencies. The moratorium ended December 9, 2024, but these provider types are still treated as high-risk: new registrants face fingerprinting, site visits, background checks and extra disclosures. AHCCCS also stopped approving retroactive registrations without documented good cause.
- Enrollment and changes go through APEP; the 2026 application fee is $750, and enrollment must be revalidated every four years.
- Services before an approved enrollment date are hard to recover now that retroactive registration is limited.
- Behavioral health residential and intensive services need plan authorization and strong medical necessity documentation.
- AIHP and TRBHA claims follow AHCCCS fee-for-service billing rules, not ACC plan rules.
Medicare rules for Arizona behavioral health providers
Medicare rules are federal, so they are the same in Arizona as everywhere else. What changes is the contractor: Arizona clinicians enroll and bill Part B through Noridian Healthcare Solutions (Jurisdiction F).
- Counselors and MFTs can bill Medicare. Since January 1, 2024, marriage and family therapists and mental health counselors (including LPCs) can enroll in Medicare and bill for their services, paid at 75% of the physician fee schedule amount.
- Home telehealth is permanent for mental health. Patients can receive mental and behavioral health telehealth at home, anywhere in the country, and audio-only is allowed when the patient cannot or does not want to use video.
- The in-person visit rule is delayed. The requirement for an in-person visit within six months before starting tele-mental health, and every 12 months after, is waived until January 1, 2028 under the Consolidated Appropriations Act of 2026. Broader Medicare telehealth flexibilities run through December 31, 2027.
- Incident-to under general supervision. Behavioral health services by auxiliary personnel can be billed incident-to a supervising practitioner under general supervision.
- Tele-prescribing of controlled substances. DEA and HHS extended telemedicine prescribing flexibilities through December 31, 2026.
- Medicare Advantage is different. MA plans set their own networks and prior authorization rules, so we verify each MA plan before the first visit.
Arizona commercial plan notes
Arizona has a large Medicare Advantage population. We verify MA networks and authorization rules before the first visit and track telehealth coverage plan by plan.
Common behavioral health denials in Arizona
These are the patterns we see most often with Arizona behavioral health claims, and each one is preventable with front-end checks:
- Services delivered before AHCCCS enrollment was approved, with no retro coverage.
- Incomplete high-risk enrollment steps or ownership disclosures.
- Claims sent to an ACC plan for AIHP members, or the reverse.
- SMI members billed to the wrong plan type.
- Residential or intensive services without authorization or medical necessity support.
Core behavioral health codes we bill in Arizona
Psychotherapy codes are time based, so documented start and stop times or total minutes must support the code billed. State Medicaid programs and plans may also require HCPCS codes, modifiers or rate codes on top of these.

Frequently asked questions
Is the AHCCCS behavioral health enrollment moratorium still in effect?
No. It ended December 9, 2024. Behavioral health outpatient clinics, integrated clinics, residential facilities and community service agencies are still treated as high-risk, so enrollment requires fingerprints, site visits and background checks.
Who pays behavioral health claims for AHCCCS members?
Usually the member's AHCCCS Complete Care (ACC) plan. Members with SMI are served by ACC-RBHA plans, and American Indian members in AIHP or tribal programs are billed fee-for-service to AHCCCS.
Can AHCCCS enrollment be backdated?
Only in limited cases. AHCCCS ended approval of retroactive registrations without documented good cause, so enrollment should be complete before services start.
Can counselors and MFTs in Arizona bill Medicare?
Yes. Since January 1, 2024, licensed professional counselors and marriage and family therapists can enroll in Medicare. In Arizona they enroll through Noridian, Jurisdiction F.
How does Healthcare Logic help Arizona behavioral health providers?
We manage APEP enrollment and revalidation, confirm the correct plan for each member, secure authorizations, code and bill to AHCCCS and plan rules, and work denials and appeals.
