Behavioral Health Billing Services in California

California runs two separate Medi-Cal systems for mental health, and picking the wrong one is the most common reason a behavioral health claim never gets paid. County Mental Health Plans cover specialty mental health services, Medi-Cal managed care plans cover non-specialty care, and substance use treatment runs through Drug Medi-Cal or the Drug Medi-Cal Organized Delivery System.

Healthcare Logic handles billing, coding, credentialing and A/R for California therapists, group practices, psychiatry groups, community clinics, FQHCs and county-contracted providers. We work inside your EHR and keep each claim routed to the payer that actually owes it.

How Medi-Cal pays for behavioral health

Medi-Cal splits mental health by severity. Members who meet the specialty mental health access criteria are served by the county Mental Health Plan, which either delivers care directly or contracts with community providers. Members with mild to moderate needs are served by their Medi-Cal managed care plan, which covers outpatient therapy, psychological testing, psychiatric consultation and medication management.

Under the No Wrong Door policy, which DHCS put in place on July 1, 2022, a member can get timely mental health care regardless of which system they walk into first. Treatment can begin while the right system is confirmed, and specialty and non-specialty services can run at the same time as long as the plan and the county coordinate. For billing, this means the screening and transition tools are part of the claim story: if a member moves between systems, the documentation has to show it.

Since July 1, 2023, Behavioral Health Payment Reform moved county specialty mental health and Drug Medi-Cal billing away from minute-based, cost-settled claiming toward CPT and HCPCS codes paid on a fee schedule. County-contracted providers now have to code the actual service delivered, at the right unit level, to be paid what they earned.

  • Medi-Cal managed care plan claims follow each plan's own billing and authorization rules.
  • County MHP and DMC-ODS claims follow the county contract, the county's CPT crosswalk and DHCS Behavioral Health Information Notices.
  • Pharmacy is billed through Medi-Cal Rx, not the medical plan.
  • SUD residential and intensive outpatient care in DMC-ODS counties requires ASAM-based level of care documentation.

Medicare rules for California behavioral health providers

Medicare rules are federal, so they are the same in California as everywhere else. What changes is the contractor: California clinicians enroll and bill Part B through Noridian Healthcare Solutions (Jurisdiction E).

  • Counselors and MFTs can bill Medicare. Since January 1, 2024, marriage and family therapists and mental health counselors (including LPCs) can enroll in Medicare and bill for their services, paid at 75% of the physician fee schedule amount.
  • Home telehealth is permanent for mental health. Patients can receive mental and behavioral health telehealth at home, anywhere in the country, and audio-only is allowed when the patient cannot or does not want to use video.
  • The in-person visit rule is delayed. The requirement for an in-person visit within six months before starting tele-mental health, and every 12 months after, is waived until January 1, 2028 under the Consolidated Appropriations Act of 2026. Broader Medicare telehealth flexibilities run through December 31, 2027.
  • Incident-to under general supervision. Behavioral health services by auxiliary personnel can be billed incident-to a supervising practitioner under general supervision.
  • Tele-prescribing of controlled substances. DEA and HHS extended telemedicine prescribing flexibilities through December 31, 2026.
  • Medicare Advantage is different. MA plans set their own networks and prior authorization rules, so we verify each MA plan before the first visit.

California commercial plan rules

California's mental health parity law (SB 855) requires state-regulated health plans and insurers to cover medically necessary treatment of mental health and substance use disorders, and to make medical necessity decisions using generally accepted standards of care. When a commercial denial cites the plan's own internal criteria, that law is often the basis for the appeal.

Common behavioral health denials in California

These are the patterns we see most often with California behavioral health claims, and each one is preventable with front-end checks:

  • Claim sent to the managed care plan when the member is open to the county Mental Health Plan, or the reverse.
  • County CPT crosswalk not followed after payment reform, causing underpayment or rejection.
  • Missing screening or transition-of-care documentation when a member moves between systems.
  • DMC-ODS claims without ASAM level of care support.
  • Medicare clinicians (LMFTs and counselors) not yet enrolled with Noridian, so Medicare and crossover claims deny.
How we prevent them: payer and plan verification before every visit, authorization tracking with renewal reminders, coding reviewed against the Medi-Cal and payer rules, and denials grouped by root cause so the same error does not repeat.

Core behavioral health codes we bill in California

Psychotherapy codes are time based, so documented start and stop times or total minutes must support the code billed. State Medicaid programs and plans may also require HCPCS codes, modifiers or rate codes on top of these.

Frequently asked questions

Who pays for therapy for a Medi-Cal member in California?

It depends on severity. Members who meet specialty mental health criteria are covered by their county Mental Health Plan. Members with mild to moderate needs are covered by their Medi-Cal managed care plan. Under the No Wrong Door policy, care can start in either system while the right one is confirmed.

What changed with CalAIM Behavioral Health Payment Reform?

Starting July 1, 2023, county specialty mental health and Drug Medi-Cal services moved to CPT and HCPCS based billing on a fee schedule, replacing most minute-based, cost-settled claiming. Providers contracted with counties need accurate CPT coding to be paid correctly.

Can LMFTs and LPCCs bill Medicare in California?

Yes. Since January 1, 2024, marriage and family therapists and mental health counselors (including LPCCs) can enroll in Medicare and bill for their services. In California they enroll through Noridian, the Jurisdiction E Medicare contractor.

Is audio-only telehealth billable for behavioral health in California?

Medicare allows audio-only mental health visits when the patient cannot or does not want to use video. Medi-Cal also covers many behavioral health services by video and audio-only, with patient consent documented. Commercial plans vary, so we verify each plan.

How does Healthcare Logic help California behavioral health providers?

We verify which system covers each member, handle authorizations, code to the county and plan rules, submit and follow up on claims, work denials, and keep clinician credentialing current with Medi-Cal, Medicare and commercial plans.
